There are three simple steps for the treatment process of stem cell transplantation at Neurogen Brain and Spine Institute whereno major surgery or incision is required for this procedure.

Bone marrow aspiration

Bone marrow is where all the blood formation takes place. It is located inside the hollow space of the bones. The simplest way to locate it is in the hip bone. This procedure is carried forward through bone marrow aspiration needle, injected into the hip bone under local anesthesia. Depending on the weight of each patient between 80 ml to 120 ml of bone marrow is drawn out. There is an alternative option of general sedation for children and adults alike too. This medical procedure is completed in a span of 15 to 30 minutes. When this procedure ends, the patient is sent to rest for three hours before proceeding for the next procedure.

Purification of stem cells

The bone marrow extracted in step one, is now taken to the stem cell laboratory, where the stem cells are separated from the remaining cells of the bone marrow. The stem cells are separated via a process called density gradient centrifugation. Mostly stem cells have a fixed density and this same property is used for separation.

Stem cell injection

The patient is taken to the operation theatre, once the purification process is done. A very thin needle is inserted into the lower back of the patient, after making the area numb with a local anesthetic and the stem cells are injected into the fluid surrounding the brain and spinal cord (CSF).Alternatively, general sedation may also be given to the patient depending on his or her medical condition. In patients with muscular dystrophy, the procedure can be a little different. The stem cells are diluted in the CSF and injected into the muscles with a very thin needle.

The schedule charted for treatment at NeuroGen lasts for six days that provides a holistic neuro-rehabilitation.The patient will have to undergo a series of tests for two reasons, as soon as the date for stem cell therapy is fixed. Firstly, to make check the fitness levels of the patient through routine blood tests like blood sugar, S Creatinine, CBC, S electrolytues, CT, PT< BT< LFT, HIV I and II anti-bodies. The second reason is to test the seriously and the nature of the condition without which planning a treatment is impossible. Special imaging tests are done as required.Direction of stem cell stimulating factors like granulocyte colony stimulating factor (Grafeel/XPHIL) needs to be managed within 48 hrs and 24 hrs before the bone marrow harvesting is done.

Day 1: Complete pre-operative assessment

A complete pre-anesthetic workup along with neurologically, physically, functionally, psychologically evaluation is conducted.This includes routine tests like blood work like CBC, Blood sugar, S creatinine, S electrolytes, LFT, CT, BT, PT, HIV I and II antibodies, HbsAg, HCV antibodies along with more advance testing to understand the severity and progression of the condition.

Day 2: Stem cell therapy

The stem cell therapy process makes use of autologous bone marrow derived stem cells which is taken from the patient’s own body. It consists of broadly three stages.

Stem cell therapy is done in 3 steps:

(1) While operating the stem cells are procured from the bone marrow through a bone marrow aspiration
(2) A process of separation, harvesting, enriching or expansion and differentiation takes place in the laboratory
(3) The transfer or deliverance of the cells through the preferred location.

Post treatment care:

The patient should rest for the day and drink plenty of fluids in order to avoid spinal headache.The next day, the patient is sent for a rigorous rehabilitation regime which is tailored to the patient’s needs.

Day 3-6: Rehabilitation Therapy

Converting a disabled person into becoming an able person needs an integrated approach. To ensure that we achieve this goal, we have a team comprising of physiotherapist, occupational therapist, speech therapist, neuro-psychologist, urologist, andrologist, and neuro-physician.

Day 6: Discharge

After the final rehabilitation sitting, the patient is advised to come back for a follow up for the next coming months and then discharged.

Follow Up

To keep the patient’s progress in track, a follow-up appointment is programmed for three months regularly.
